One thing I noticed is that the album was published in 2006, 25 years after HIV/AIDS was identified in 1981, so my interpretation of this had mode to do with the HIV epidemic and how it's severly impacted Africa, but much less so western nations. Of course, lots of things have been happening in Africa for approximately 25 years, and "infection" is just as likely metaphorical.
