I think you guys could be taking the title too literally. Affinity can refer to a relationship or marriage. Leper, here, could be used as an adjective describing a relationship. Here, the title could refer to a messed up, abusive, falling apart relationship, instead of actual persons with leprosy.

And, looking at the lyrics, murder + necrophilia would fit nicely.

The main character here takes someone he cares for to a body of water and drowns them. Perhaps he believes he is somehow doing something good and setting this person free.

"All over now, forgotten why I needed this
Standing down disappear into the obscure
Resting days waiting for new disease
Biding time locked inside insanity"

The deed is done, but he's forgotten why he needed to kill. The ordeal has scarred him and eventually drives him into insanity. This murder is always burning at him, probably thrusting him unto a state of regret and depression.

The first verse could be about some huge idea that's suddenly comes over him. "From nothing to a life code" -- this idea has given him a sense of purpose, creating some need that drives him to kill this person he loves.

If any of you have seen the movie Inception, this insight makes me think of the part Leo Dicaprio's wife kills herself..

The entire song takes place in a recurrent dream (more precisely, a nightmare.) The dreamer is searching, trying to discover some meaning in this. The "old memories" could mean that the dream is rooted in some repressed, traumatic event from his past.

He hears the the distorted laughter of familiar children -- they seem to be dead. He sees his reflection in a pool of water; he is covered with "rotten earth and insects", like he's been dead and buried.

"Bleeding animals in a field of fire" -- some serial killers started out with things like torturing and killing animals or starting fires before they moved on to killing people. This could be the dream of a serial killer. It would explain how tortured his mind is to have these tormenting nightmares.

In the end, the morning comes and he wakes up.

If this is a concept album, this could connect with the next song, Deliverance, which seems to center around the character drowning someone.
