Your post was probably the most helpful comment I have ever seen on the internet.
I've been following Tom Carefully for some years now and I still haven't got to all his albuns. Its a slow process followed by the purchase of his LP's. Not many people understand what Tom Waits is saying but its good to find someone that can.

She could be just smiling. When Tom Waits wrote down "All night long on a broken glass" you think he meant the glass was really broken or someone was sipping way too much booze?
I can go years without listening to REM but every time I hear this man's voice I guarantee its gonna be playing in the house and car for at least a week... as many albums as I can.

I never realised this song was so awesome. I guess its never late.

I have to think about what he means with it's in the water.

I think he might be saying the oposite, that it really is a special way and its not just FUCKING and just uses the work "fuck" to contrast with the "special way" thus enhancing it.
This is a long shot, but could have something to do with the heroin. Heroin addicts are complicated they lose respect for people they love and seem to lose all sense of humanity, when they need it badly. Maybe the "fuck" slips out in one of these phases. (haven't thought about the writer not telling what happened to someone, but what is happening to him, but dont believe it very much so ill leave for later).

About "tap the vein that bleeds" u dont that a vein for it to stop bleeding, you tap them to find a vein. If you already injected yourself with anything you wont keep tapping the vein.
Unless you're addicted for years and have trouble finding a vein, cause heroin does that. If he's having trouble finding a vein and missing veins he will keep tapping for veins.
But dont think that anywhere in the song it suggests that it's something going on for a long time, but doesn't say it isn't 2.
