I believe this song is about the cards that people are dealt in life, and how unfair the results of chance can be on people, because of how terrible a hand most people draw.

The chorus questions what the difference between different lives really comes down to: "And that's when it hurts, When you see the difference" - being confronted with the reality that the only difference between you as a human, and another human in a better or lesser position, is in the blood. The change in the second refrain is about how much easier some people have it in life.
