For the longest time, I too thought this song was just about werewolves, and I knew the lyrics inside and out, lol. I guess its just a testament to how this song is both very subtle...yet really obvious if you think about it the right way. Extremely well crafted lyrics.
I think this is a beautifully sensual song. I've since come to just see the werewolf elements as just being metaphorical for the empowerment a man recieves from sexually gratifying his lover. In this song, the woman is not asking for his help...he's willingly offering himself.
There seems to be a few elements to indicate that the man is performing oral sex on the woman not only because he is turned on by it, but also because she is having pains and sex/orgasm can provide relief for that. Note "we'll alleviate the pain...let me kiss the curse away".
However, if you understand Peter Steele and his song writing style, my guess is that he probably decided to write this song because he thought it would be funny to write such a sensual song about menstration cunnilingus.
