I don't think this is about a woman who is just finding out about her cancer. I think this woman is very close to dying. I picture a woman laying in the hospital bed just before she passes and is having these "dreams," only they aren't really dreams so much as just her slipping in and out of consciousness. The part where the doctor is driving the limousine makes me think that she is kind of looking around the room and just seeing the people in the room in her dreams. Looking at her kids one last time as they sit in the room and play... still a very very sad song and I love every drop of it.
