I think the concept is Freddie having conflicting thoughts, he is wanting to confess his sins, but the shouting voice is saying "Liar" almost like a voice in his head accusing him. I think it is this voice that wins out over his own confession and asking for help, as you can see from the end of the song. It is a really aazing concept, I think it is supposed to be a similar concept to Eminem's "Guilty Conscience," with the two voices inside you arguing.
