"This is the scent of dead skin on a linoleum floor" abuse at home? Possible... maybe just the familiar smell of a hospital.. "regular decorated emergency" the decorated means "wasted" as far as I can gather and the references to "fighting" are to do with fighting this routine of going out and getting fucked up on pills.
