He's talking about how he loves her so much ("I absolutely love her when she smiles").
He keeps telling her that eventually she'll feel better, but it never really happens ("how many days in they year have you woke up with hope but you only found tears? I can be so insincere, making you promises, never for real"). She has low self-esteem and basically waits around making excuses for why that is. And finally, I think this song represents him coming to some self-realization and turning point regarding his non-committal approach.
Based on the refrain, I think that the guy is very irritated at the way his girl friends handles things. He thinks she's emotional enough for everyone in the world, let alone just one girl. And it pissed him off. But then they're are those good times, the ones where she's smiling that huge smile that's big enough to light whole world, and he realizes that its all worth it.
