    Jeff is from Montana and I think Mccready is as well. 2 of the 2 shows that I have seen of theirs (They Play a show in Missoula Montana everytime...it is Jeffs hometown) Eddie dedicates the song to someone (multiple someones) in Big Sandy Montana. I know it was either Jeff or Mike that lived in Big Sandy, but dont know if that small town has any connection to the song.

    I dont think it is about his mom, I think it is about an old woman seeing an old friend and having that rush of emotion over seeing someone that she really cared about but never expected to see again...and the bit of hope that it stirs.
