One of the best Megadeth songs. Surprised it didnt make it on the Greatest Hits album.
Easily interpreted as a battle with drugs. But! The loss of a love certainly appies. There is certainly an undertone of sadness/depression.
More specifically, I think this guy has a chaotic personality that won't leave him alone. This gets in the way of his love. She knew it was going to happen, told him so. And one day it happened. She left, or killed herself. Then he was by himself to face the world while fighting throughts of suicide.
It refers to drugs, of course. I think I first met the expression in William S. Burroughs's Naked Lunch. It is explained, that when you're addicted, it's like there's a monkey on your back that you have to feed constantly, or that wants a bit of everything you own, thus making your life harder. I'm not sure but it might as well mean that the monkey is pretty funny and entertaining for a while, but it wants more and more and gets more and more annoying, since you can't get rid of it.
The bottom line, it's definitely a metaphor for drugs.

That's talking about an addict who had a friend that was helping him through it, and starting to turn him around. Suddenly, his friend died, and now he doesn't know what to do.
