I believe this is about someone who's in a relationship with someone who they may still love, but the battles between them and this person are getting to be too much. It sounds like they have their own issues with their own life as well, so the mix of relationship troubles, arguments combined with the other issues in their life have come to a head. They need to decide if it's worth sticking it out or if they should just speak their mind & perhaps just end it once and for all.
