If it were about eating disorder or fueling your body, why do you think she would be feeling free from that?
You will never feel free from eating, anyone with an eating disorder will tell you that. Eating is actually something those people normally don't want to do.
Looking at it from that point Gentlemen logically it cannot be about eating disorder. Unless of course she plans to commit suicide by "I run over my own body with my own car" However I kinda doubt it...
and the end is her feeling free because she is dead.
but i think the "i run over my own body" line is more about how she created the problem she is talking about in the song.
1) The lyric is "do or die" not "a newborn dying"
2) "Running over my own body with my own car" is a metaphor for how people are literally killing themselves by overeating junk foods, but it also captures the vicious cycle of guilt that, in turn, prompts the overeating (like how some people crave sweets when they're stressed or depressed)
3) "A baby's cry", the only line in the song that makes an overt reference to children, born or in utero, deals with a baby's response in reaction to her, not the unjust slaughter of still-forming embryos.
4) The use of "feelin' free" is an ironic statement. The protagonist is a slave to her cravings, but feels great once these cravings are satisfied, as with any addiction. Thus, she/he 'feels free' even though we, the objective listeners, can see she is not.

well I don't know. Taken semi-literally with semi-lucidity I.. Guess about eating disorders? In this case too much rather than too little. Pressure to be skinny in the city?
