I know it isn't written about him, but it really sounds like it!
A Scumfuck is supposed to be a nickname for a grotty punk who sleeps on the streets and does hard drugs like heroin.
I think the last verse is what the scumfuck/heroin user thinks, whereas the other verses are the thoughts of the band members. I think the heroin user is trying to escape from something in his life/past with the drugs.
