The song is about a drug addict. The moment of surrender is when a drug addict admits he is addicted.
Just because it has the word 'horse" in the lyrics doesn't mean it's about heroin. If that was the case there'd be many little kids sitting around the classroom or the bedroom hearing about taking a ride on the drug:) I believe the song could be about any realization. When you are held captive by anything (it could be a gambling addiction; an addiction for bad guys; an addiction to wasting money on stuff; etc) you are not free. Once you're able to acknowledge your problem (addiction), only then do you have a chance at true freedom. I love the mention of "stations of the cross." I'm guessing one or more of the band members is Catholic...or at least Catholic-knowledgable.
It is about a drug addict, Bono even said it himself. He wanted to take on a role during the song--its not about him, but in someone else's perspective.
