The character is seeing dreams of how he and his girlfriend will change as they grow older and his close-to-perfect relationship coming to an end. Later it is revealed that the girl is afraid of the same thing.
"Is this what it means to be free?" is all about this idea: that there are no metaphysical forces shaping our destiny to be glorious, just slow decay. In the end of the song, their fears have taken over and THAT is actually what finally destroys the relationship. Illusions of perfect happiness fade; the premonitions come real, but only because the couple caused it themselves by worrying about it.
Anyway, I have a bit of an odd theory about this song (I don't really believe this, it's just what it reminds me of). I think Joey wrote this song about an experience he had taking some kind of hallucinogenic drug. What really gives me this idea is the lyric "today all the illusions are fading away, I've drawn my conclusions".
Anyway, it sounds like this song is about people who follow the same daily routine for their entire life, and that they're very predictable, and the singer is saying that he's "been to the future" because he knows that they won't change their habits and they'll always continue on that way.
to me the song is definitely about unwillingness or inability to change.
