So true though. If you have the cash, people can do just about anything for themselves. Doctors lie about the truth behind unecessary surgeries all the time.. pretty much for the cash haha.
I think its about a girl who wants to be perfect, or atleast close to it, and she wants to get an operation, and, once the operations finished, he's afraid she'll go for something much better to him, or, if she stays with him, he'll leave her, cause he liked the imperfect in her,. Now that its gone she's just another fake girl like everyone else, and he'll leave her. The part where it says "i'd rather see you dead, than live without me" that's where he would rather have her dead, during the process, than him leaving, and her wanting more of him.
this part "Gather round, gather round," etc, is where the death of the old her, and when the operation was finished, (the resurrection), its like a deformed zombie of what she used to be,
"she'd rather see me dead than face me" that probably means, she'd rather have him die, than hear his lecture, about how he doesnt like the operation.

hes in love with a girl that is drunk with the pressure fame brings. She is looking to change herself to fit a "perfect" image. He is sickened that she would do this. He thinks she is beautiful as she is and is in love with the real girl. He knows he wont love the fake, plastic girl that was altered, and he is a bit insecure about her feelings for him. If her physical appearance changes, what she really wants will change and shell be more interested in someone with similar alterations....

"I don't think you'll ever want to love me" -- This girl is pressured into refusing to love someone as imperfect as this guy.
"You'd better listen to your doctor" -- Because this doctor is going to help her achieve "perfection", he's giving her the mindset that anything less than perfect in unacceptable. Therefore, the guy and girl could never be together.
"If the dollar is right" -- The doctors don't care about her really and how she's being brainwashed, just money.
"I'd rather see you dead" -- He would rather her die in the process of perfection or die imperfect than live with her rejection.
"I'm in love...but not for long" -- He won't love her after she's been changed.
