The obvious theme here is a woman/relationship, but I agree with a few others here that think it can be applied to anything that someone loves that hurts them. Drugs ring a bell here. A few lines in the song are very close to normal drug references and/or their effects on a person.
I put up a comment on Genesis - Tonight, Tonight, Tonight with the same sort of vague interpretation of being addicted to SOMETHING. The songs are closely related and were blatantly pointed out to me by the Kleptones mashup containing both songs :)
To me this is NIN's My Sweet Prince. Yes, I prefere With Teeth, and it has a lot more to offer when it comes to power, intensety and emotion, but they're both often mistaken for love songs (I compare With Teeth to MSP and not the other way because I found MSP earlier, not because it's better). Now I don't really find this strange, as love, in many cases, is just as easily addictive as drugs, and almost easier to abuse. As Trent is a genious, I don't think he was oblivious to this, and used "she" instead of "it" so more people can relate to what he was going through. Now I don't claim to know anything about losing a drug habit, but I do know how fucked up you can get on love, and I know how hard it is to quit. To me, being in a relationship ment not having to be alone with myself. You never have to handle anything on your own, and the world seems less terrifying. Hell, while in a relationhsip you don't even have to love yourself, the other person does. What I'm talking about is the fear of facing the world alone, and while in a relationship or on drugs you don't have to (well, while on drugs you don't have to face the world at all). Now I'm not saying that this song is about fearing the world, because it's obviously about how hard it is to get out of a drug habit. It's holding on to you with teeth.
Another absolutely genious thing about this song is how it's devided into two extremely powerful bits with one weak and vulnerable part in the middle. To me it seems that the vulnerable part represents how weak Trent felt while realizing he had to quit, or while quitting, and the intense parts represents the power the drug had over him. The last part might represent both the power of the drug and his own though, it could be a real combat scene for all I know.
Still, this song is incredibly sexy.

He's saying how she's the hottest girl (in all sectors) he's ever been with and at the beginning everything seems really good. It seems like there's this no strings attached feel to their relationship and she's being mysterious with him and thats turning him on too. The 'With Teeth' part is about how she's got ahold of him and he likes it.
Then something in him makes him want to stop their 'thing' maybe he has an actual real girlfriend that he's with and that seductive girl he had on the side made him 'wave goodbye to what he was'. When he says 'i cannot go through this again' i think he's feeling guilty and he's trying to end ties with that 'other' girl and she doesn't wanna let go.
Or maybe in his head all the memories he has of that girl won't go away and he dreams of her. It could be that this past girl keeps haunting him in his dreams and he wishes he still had her because she was perfect in his mind. Maybe dreaming makes him say 'i cannot go through this again' he misses it all too much.
