i see it as a relationship and the guy is so in love with the girl but she's kind of unfaithful and she's just always messing with his head but she's still in love with him. and she comes to realize that she needs him but then just gets caught up with emotion and can't handle it and she runs and he is desperate for her.

basicly it relates to the relationship between Dagny Taggart and John Galt saying that they are the people who truely belong on earth that now is run by the very people that exploit the very thing they live on
wich in reality suggest that their whole purpose is to destroy the earth because they are those who are not willing to think
and he is saying to her that the cuts feel real nice because it is his way of saying he has to cut his arm open in order to let the poison drip out and before things can be right and life can be lived as it should she has to accept the fact that the people who must die MUST die that she may know it but now she has to actually try and have no pitty on the men who asked for death instead of life.
