Hear the screams of my world through the rain." - the narrator knows that the person he feels very close to has to go away and not let good things pass them by, but they must feel united even if they may feel hurt that they are separated, and hope to still be able to communicate.

One day we'll be back for one moment in time, endlessly searching for you." - may signify the storyteller is in the fields where he used to spend a lot of time with the person he is talking about, but now that they're gone, he feels sad over that.
