I agree with TheEnemy Within, but I think that Max is reflecting on a past relationshp, be it with a friend or someone who is more than a friend. I think that that he feels betrayed by her but also thinks that she is going to have it worse than he does becuase she has to live with the consequences of her decisions.

i think it's about an ex that left max to pursue more self-destructive relationships. she left max and is now having one night stand after one night stand. i think the knife is going into her, not max, and she is helping them (all of her new partners) sink it in.

saying that even though she's treated him terrible, both in and out of their relationship most likely, he's still there for her.

I definitely believe this is about a girl from his past that wronged him. I think "I'll be there" is saying he will always haunt her. Maybe he's hoping she will always have to live with what she did to him.
