like maybe refering to the same girl as the first part of the song that he cheated on and then they broke up and she's back in town now and "drunk at the bar"
also this would further explain "what's worse" about his ex being back in town is that he knows if he can go see her something will happen between them again to re-surface further complications.
i have come close to this type of failure in very similar situations with previous relationships but luckily have avoided most of them. great way of putting it, Tim.
