    YES - “A Shot In The Arm” is a cliched phrase meaning “I need some inspiration, some ‘get-up-and-go”.

    ALSO, yes, the most efficient way to use drugs is intravenously, as Slash once said, “after you’ve injected a drug, any other method of ingestion is just INEFFICIENT” lol…in other words, giving yrslf “A Shot In The Arm”

    So, the singer is needing “A Shot In The Arm” for inspiration - or maybe just escape…and he might just believe that the “shot in the arm” he needs is, well, A SHOT IN THE ARM…

    Again, he will give himself a shot in the arm because he needs a “shot In the arm”

    Especially if he’s already dependent/addicted. When you’re in opioid withdrawal, the ONLY THING THAT WILL WORK TO MAKE YOU EVEN FEEL LIKE MAKING THE EFFORT TO EVEN BREATHE…

    So - and I wouldn’t think I’d need to explain this to anyone on the net searching for the meaning of a song - but Tweedy employs a DOUBLE ENTENDRE. “A Shot In The Arm” has a double meaning here.

    Along those same lines, I’d like to comment on the lyric:

    “something in my veins Bloodier than blood”

    Blood FEEDS our body/organs the “nutrition” it needs, such as oxygen… (hold that thought).

    A person in a depressing relationship/situation, or even just someone who feels down-and-out, the way we all do sometimes, feels like he needs something MORE than just to keep himself alive, he needs something that makes him want to even BE alive…

    “Something in my veins Bloodier than blood”

    Meaning, “something that changes my outlook in an instant from suicidally-negative to ‘Life is just awesome’, and FEEDS MY DESIRE TO BE ALUVE IN THIS MOMENT, and whether this is a “good” development or not is left up to you, but this guy sure does feel like he NEEDS something “more”…

    Been there, more times than I’d care to think about, and I don’t expect I’ve seen the last of those moments. To quote two other amazing songwriters:

    “You gotta take ‘em, baby, as they come”

    and

    “HELP!”

    As well as…

    “Whatever gets you thru the night… Whatever gets you thru yr life… Is alright. It’s alright. ???? “
