It's almost like he is explaining to them how life is going to go and how to not take things too harsh and to live to the best of your abilities because you only have 100 years to live.
Maybe it's someone he's trying to be like.
The "she" thus refers to another girl he fell in love with. He's happy with her but love never fades as the first girl is still very important to him.
Finally, I think he hopes she's achieved happiness like he did. Perhaps they no longer speak to each other or he's lost her over time. Perhaps she died when they were in love at 15. But, wherever she may be, he thinks about her moments in his life and he never forgets her. He's happy they had the experience they did in his 100 years. Now he's happy with a wife and a family.

Whenever I hear this part, I think of a barely concious old man who is dying in a hospital. He's all alone, with no family or anything near him. I always picture him looking up at the ceiling, "counting the ways to where" the love of his life is. I think he loved her as a young man but didn't do anything about it and then she moved away or married someone else or something--basically just slipped out of his life. He's spent all his idle time trying to figure out a way to get in touch with her again, that's what "counting the ways to where you are" means. Now he's "dying for just another moment" (beautiful phrase, by the way) and he is thinking about her for the first time in awhile, as he lays there all alone in the hospital, only one minute left in his life, he just dreams his last breath away for the one he loved a long time ago.
Just my silly interpretation, and I haven't even begun to convey the images this song brings up. I think it's one of the most beautifully written songs I've heard.

Basically- this song means life is short and death is inevitable, so you have to appreciate life as it is and worry more about today than tomorrow.
