I do not believe the first and second verse are the same story. The fact that he uses "he" in the first verse "you" in the second pretty much confirms this. I'm pretty sure the second verse is about what it might be like for you to be in a drunken car crash. Imagine yourself waking up bloody and dazed on the street, people are screaming, you see your friend somewhere in the wreckage with blood pouring from their head. "get up, get up, please start breating" is probably what you would be saying.

mabye she was just some girl that he (the character not tom) got drunk and came back to a building where they had sex....the next day she wanted to see him again and he didnt want anyone to know that he banged her so he killed her....and for the rest of his life he wont be able to sleep at night because he knows shes waiting in his nightmares :D
