Had to throw this out there... this thing reminds me of the introduction to Nine Inch Nails - Mr. Self Destruct off of The Downward Spiral. I think LP said once they were partially influenced off of NIN, so I wonder..

Actually, I did find a meaning to this song, and probably a very good one. What if, glass being hit with a hammer is like someone being hit by the lies/backstabbing/ill-treatment/etc of someone(s) else? Then it would suggest, that you can only keep on "hitting" someone until they eventually break (that is, the glass breaking at the end).
A lot of Linkin Park songs seem to talk about being pushed a more and more, eventually until reaching the point of breaking (From One Step Closer "everything you say to me takes me one step closer to the edge... and I'm about to break"). This seems to decribe just that - with hitting the glass until it eventually breaks.
Also, like how it goes nicely into Don't Stay. As for the "extra tap", I don't know, but what I said above is what I think Foreword means. After all, that is the message that LP seems to put across in some (if not most) of their songs.
