"Brother, it don't matter," might be a reference of one positive man propositioning another, effectively saying that since "I'm a vampire, you're a vampire too" it doesn't matter if they get together as they're already infected. "Do what you want and then I can do it to you" is clearly a line about sex.
It may even be about someone who wants to be infected; believe it or not, some men in the gay community consider it liberating to possess the HIV virus and actually set out to acquire it.
