Clearly this song is about someone dealing with the aftermath of a break up, and coping. Petty here explains that his coping method is to go on a drive and smoke while blasting his speakers.
And while I think that everything I have just said is nothing new and a few others have pretty much already spelled all that out, I want to address the line that gives us the name of the song. "You don't know how it feels" There were some comments made about how this line is whiny, or how it's a lame line or something, but it's a crucial part of the song where the real meaning comes through. Petty is justifying his coping mechanism and telling people to stop judging him. Which I think is a very important point to make for someone going through a break up since immediately afterwards, everyone and their brother is chiming in about how you should be handling yourself and throwing out all the other stereotypical lines like "Oh, come on sweetie. Perk up. You deserve so much better.."

next it is about, my opinion of course, smokin when you feel more dead than alive, when you feel trapped inside your own body looking out hoping one day to be free. to never get the chaance to experiance life only live it. To have so much pain inside you do not think about ti just cover it with weed, then let the good times roll and trying to be "happy" for if not just one minute. with friends, or alone.... then start to remeber your life you burried in side flying down the high way with nothing but the drone of your engine and the twinkle of distant headlights. . . . in the moon light,
