You were the same as me but on your knees- Perhaps he's a "casual user". He does heroin once in a while, making them "the same", but she's completely addicted and brought to her "knees" by the drug. And to me, it's always seemed that he'd been trying to distance himself from her (almost fell into that hole in your life, and I'll go on as you get colder) He cares but he's cautious that her addiction may have a negative imapact on his life. Are you someone's prayer= He's saying that there's someone else out there praying for someone like her to come into their life. And I've always thought that at the end of the song (I'll lead you home) he got her into some sort of rehab program against her will. She hated him for doing that to her, but in a way she loved him for caring for her. He had the same sort of love/hate for her, hence the "I'll become what you became to me".

That's just what the songs allways seemed to say for me.
